Certificate of Insurance Request

A certificate of insurance (COI), also known as an Accord 25, is a document that provides policy details on your insurance coverage, verifying  limits of coverage, the issuing insurance company, your policy number, insurance limits, the named insured, and the policy’s effective and expiration dates. It also provides information specific to a unit, including owner name and mortgage info (if applicable).

In an effort to continue providing superior service to our clients, we are using a state of the art insurance certificate delivery system that provides certificates on-demand. Please read the following instructions carefully before submitting a request as any mission information may cause a delay in your loan processing.

Certificates of insurance are automatically sent annually to your Mortgage Lender.  If you or your Mortgage Lender has not received the certificate, please visit, For help, please call (833) 237-8123.

For those that need a change to their current certificate due to a closing or refinancing, please visit,, Certificates are available 24 hours a day, 7 days a week, 365 days a year.

Please have the following information available as this information is required by your lender:

  • Association name

  • Unit address and unit number

  • Loan number

  • Mortgage company name and address

  • Mortgage clause: ATIMA (as their interests may appear) & ISAOA (its successors and/or assigns)
